Equipment Loan Application
Equipment Loan Agreement of Extracurricular Activities Division, CJCU.

Approved on September 05, 2017, by the EAD, CJCU, at its 2nd division meeting
a. Who can use it: School clubs & student associations of CJCU have priority of use; Administrative & educational units come second.
b. Duration of use: According to the date of activities on application form.
c. Equipment Loan:
- Please bring along the application form & register at the EAD office 3 days before the activities & confirm the amount of equipments loaned.
- Fill in the borrowing date & return date in order to avoid overdue.
- Please cancel the reservation 1 day earlier if the equipment is not going to be used after registered. Borrowing rights will be suspended for a month if it is not informed.
d. Equipment Pick-up:
- Pick-up time: Monday to Friday 13:30-16:30
- Check the quantity & test the performance to check if the equipment works normally. If any damage occurs after lending out, the borrowing unit has to bear the responsibility of compensation.
- Student card or staff card is required when equipment is picked up. (not deposited)
- The use of equipment is limited to the activity areas & the clubs reported, not allowed to be used at other areas & lent to any unit or anyone. Violation of this regulation will result in a prohibition of equipment loan for 3 months.
e. Equipment Return:
- Return time: Monday to Friday 09:00-11:30
- Clubs & associations which do not return the equipments on time are required to perform service hours. Each overdue equipment requires 1 hour for the first day, 2 hours for the second day, 3 hours for the third day, and so on. Those who do not finish the service hours will be prohibited for equipment loan.
- Adminitrative & educational units which do not return the equipments on time will be prohibited for equipment loan for a month.
f. Equipment damage & loss:
- Loss of loaned equipments requires full compensation.
- Damaged equipments will be sent for repairment and identification. Borrowing units will be responsible for compensation if the equipment is identified as man-caused damage.
- Borrowing units which do not compensate for man-caused damage & loss of equipment are prohibited for equipment loan.
- Man-caused damage & loss of equipment occuring twice in 1 year will result in prohibition of equipment loan for 6 months.
