Application for Activities

On-campus Activities
- To apply for activities on campus, fill in the related application form in the student portal (http://eportal.cjcu.edu.tw/student/home/), submit and print out. Hand in the activity proposal (including title of activity, date & time, activity content, attendances, venue) for review, according to the sequences on the application form.
Hand in to the EAD for preliminary review:
3 working days before activities if NO equipment loan needed (excluding holidays);
5 working days before activities if equipment loan is needed (excluding holidays),
Late applications will not be accepted. If equipment loan is needed, the equipment loan application form can only be submitted after the activity application is approved. Please refer to the Club Event Equipment Loan Application for more details.
- In need of practising fire dance due to activities, please fill in another Fire Application Form (attach to the activity application form). Also, there must be full assistance & oversight of a instructor on the activity day to ensure the safety of students.

For off-campus activities, documents needed include insurance certificate, attendees list, parents' agreement, tour bus drivers license, tour bus business registration certificate, tour bus contract, tour bus check list. Refer to the attached form for more details.

For tour bus rental during off-campus activities, be sure to check whether the vehicle escape facilities can be used normally & require the tour bus driver to play related videos about safety facilities. The Directorate General of Highways (DGH) provides convenience to people on the Motor Vehicle Driver Information Service webpage. People can easily look up to related information about tour buses that comply with safety equipments by just entering the license plate number or the company's name.
