Club Activity Equipment Subsidy Application

The Ministry of Education Rewards the Development of Private School Affairs - Club Activity Equipment Subsidy Application 

Please note the following & the receipt date when filling out the form.

Notice for filling out the application form:

1. Applicant can fill in a single club/more than a club as a unit;
2. Each equipment must be filled in individually and selected in sequence;
3. All item prices must be filled in according to the supplier quotation. Please do not fill in the prices according to internet or any department store. Quoted prices must be based on the ones quoted by suppliers located in southern county for the convenience of the school purchasing group to purchase and for follow-up maintainance;
4. All equipments filled in will serve as a reference for the EAD while purchasing club equipments subsidized by the Ministry of Education;
5. Digital products (such as computer, disk rewriter, etc) and photographic equipments will be unitedly purchased and safekept by the EAD for the use of all clubs according to the gross requirement amount of the current academic year;
6. Please do not fill in portable speaker, megaphone and platform trolley as these will be collectively purchased;
7. All relevant fields must be completed or it will not be accepted. Any questions please feel free to ask at the office of EAD;
8. Completion date of the semester: From now on until February 28;
9. Please attach the orignals or the scanned copies of the 3 different suppliers' quotations. For details of the document's subject, please refer to the descriptions.
10. All purchase applications will be collected & organized by the EAD and submitted to the committee for review.
11. Reviewing standards: a. The annual evaluation results of the last 2 years; b. Whether the clubs, as required, participated any workshop (Newly-Elected Club Committee Training Workshop or any training workshop required as announced) and regular gatherings (work report); c. The usage & maintainence of equipments purchased applied in the recent 3 years (usage log records required for checking); d. Whether the clubs activiely assist with every school activity.
12. Review Committee: Comprised of the Dean of Student Affairs, the Director of EAD, club counseling instructors and student representatives.
